What do I do for after hours clinical care and in an emergency?
Unfortunately, since this is a boutique solo practice I am unable to provide emergency or after hours clinical care. For any non-emergent issues, you will have the ability to leave me a message and/or send me a message through our secure portal. You may also send a text to the messaging service but please note that text messaging is not secure. Please allow up to 72 hours for any messages to be answered. Note that I cannot always provide clinical care or medication changes over the phone. These issues will need to be addressed in person at a follow up appointment. By signing this agreement you acknowledge that you understand that any after hours emergencies or urgent clinical concerns will require you to call 911 or go to the ED.
What is the process for Medication refills?
Please note when you will be out of medication and make an appointment PRIOR to running out of medication. I cannot refill 30 day supplies of medications after 3 months without a scheduled follow up appointment.
What Prescriptions will the doctor provided?
I do not provide prescriptions for any controlled medications outside of ADHD medications. This includes prescriptions for benzodiazepines. All prescriptions will be sent electronically straight to the pharmacy which you provide for your file. Please note that I do not have the ability at this time to issue paper prescriptions.
Do You Take Insurance?
We do NOT take any insurance, Medicare, nor Medicaid. The process of billing insurance/Medicare/Medicaid is arduous and time-intensive, and requires practices hire professional medical billers to handle all the paperwork. By not accepting these third-party payers, it allows us to keep our rates low. If your insurance company will reimburse for telemedicine visits, we can supply you with a Superbill you can present to your insurance company.
How Much Does An Appointment Cost?
At the time of your appointment, you can select to see Dr. Barczak or one of his nurse practitioners. Some wonder if they will receive the same service and expertise if opting to see a nurse practitioner. Dr. Barczak reviews patient notes and medications prescribed for every patient. So you can rest assure Dr. Barczak will always be a part of the treatment team even if you opt to see one of his advanced psychiatric nurse practitioners at a reduced cost. Cost to see Dr. Barczak is $265 for a new patient evaluation and $165 for a follow-up appointment.
Is there a No-Show fee for missed appointments?
We do have a strict No-Show policy and you will be charged half the appointment fee if you are a No-Show
How will I receive my medications?
All prescriptions are sent to your preferred pharmacy electronically via an electronic medical record.
Do you use Skype?
Because we take the utmost care to protect your information, we do not use a non HIPPA or uncertified platform like Skype. We have a HIPPA secured platform that allows us to exchange information in a patient portal.
Do you fill out paperwork for disability, FMLA, or other?
Paperwork is filled out on a case by case basis. While we try to assist in any way possible, remember there is a cost associated with filling out paperwork with a minimum charge of $80. This is to account for research and time needed to prepare your documents.
Do you perform evaluations for court?
Because evaluation for court cases are often time consuming, Dr. Barczak and team does not perform any forensic evaluations. Additionally, because many evaluations require psychological testing, this will need to be done by a licensed psychologist, often in an office setting. Dr. Barczak and colleagues assess for medication management issues and do not provide advice for anything court related.
Where does Dr. Barczak see patients?
Dr. Barczak is licensed in Georgia, Kentucky, and Tennessee.

Should you wish to contact Dr. Barczak between scheduled sessions, you are encouraged to call, (502) 230-1065, or email, info@BarczakMD.com . Dr. Barczak or allied staff will make every attempt to return messages within one business day.
Should you experience a life-threatening emergency, call 911 for assistance. You should then notify Dr. Barczak. Every effort will be made to return your call promptly.

Schedule and pay for your appointment |
By visiting the "GET STARTED" tab and selecting "SCHEDULE APPOINTMENT" you may follow the steps to select an appointment, complete complete registration and pay for your appointment.

Complete you medical history information |
Once your appointment is scheduled you will receive a confirmation email with log-in instructions for the patient portal. The patient portal maybe accessed through Google Chrome or apps available from the Google Play Store or Apple App Store. Upon receiving the instruction complete the required medical history information and designate your desired pharmacy.
